高脂血症患者之超高頻聽力喪失

Ultrahigh Frequency Hearing Loss Patients with Hyperlipidemia

摘要


將75名因耳症而前來就診的高脂血症患者,依其血液生化檢查結果分成三組:A組選取血清總膽固醇值高於240 mg/dL者,計25人(50耳);B組選取血清三酸甘油酯值高於150mg/dL者,計25人(50耳);若兩者皆高時,則納為C組,計25人(50耳)。使用Beltone 2000型聽力檢查計進行傳統及超高頻聽力檢查,結果:A組患者28%之受測耳傳統聽力圖異常,而76%之受測耳其超高頻聽力呈現異常。B組患者20%之受測耳傳統聽力圖異常,而80%之受測耳其超高頻聽力呈現異常。C組患者32%之受測耳傳統聽力圖異常,而有76%之受測耳,其超高頻聽力呈現異常。以上數據顯示,高脂血症患者雖其傳統聽力圖正常,然而約50%病人其超高頻聽力業已喪失。因此超高頻聽力檢查,有助於輔助診斷高脂血症所引起之內耳症狀。

Seventy-five patients with hyperlipidemia and inner ear symptoms were divided into 3 groups according to the serum lipid levels. Group A comprised 25 patients with serum total cholesterol level greater than 240 mg/dL. Group B comprised 25 patients with serum triglyceride level greater than 150 mg/dL. Group C comprised 25 patients than 240 mg/dL and triglyceride level greater than 150 mg/dL. Another 25 persons free from ear symptoms served as the control group. Evaluating the hearing level for air conduction in the study groups, 28%, 20%, and 32% of ears showed abnormal conventional audiograms in the A, B, and C group respectively; while 76%, 80%, and 76% of ears revealed impairment in the range of ultrahigh frequencies. These data suggest that more than 50% of hyperlipidemia patients suffer from ultrahigh frequency hearing loss although their conventional audiograms are normal. Thus, ultrahigh frequency audiometry may serve as a useful tool in assisting the diagnosis of inner ear disorders caused by hyperlipidemia.
