多頻道人工電子耳植入狀況的X光攝影評估

摘要


術後X光攝影判讀,配點聽力檢查,聽能復健與語言治療效果評核,乃是人工電子耳術後成效評估與日後研究改進的必備檢查項。台南奇美醫院耳鼻喉科,從1990年到1994年間,選擇重度聽障、無法以手術或助聽器改善之個案共11例,其中成人6例、孩童5例,進行22頻道人工電子耳手術,及術後聽能訓練與語言治療。所有個案都於術後採行傳統之史坦福氏 (Stenver’s view) 及修正後之耳蝸式 (cochlear view) 平面X光攝影,判讀其中電子耳之植入狀況;並與術後開機之頻閾圖示 (mapping) 結果作比較:證實使用此種平面X光攝影判讀,可以了解多頻道電子耳植入電極徑路深度、位置與數目、探測是否有電極線路扭曲、推斷是否有電極術後滑脫等現象。這些客觀的植入深度與電極數目,相信可據以分類電子耳術後病例群,有助於深入研究及評比各種術後聽能練方法和其成效,造福更多聽障患者。

Postoperative plane X-ray films along with psychoacoustic examination, auditory training and speech rehabilitation are necessary in evaluation of the implant results, and may be helpful for the future research and development of the multichannel cochlear implant. The plane X-ray films in cochlear implant patients may reveal the intracochlear electrode position, detect the possible electrode kinking and provide a reference if postoperative slippage occurs when the implant child grows up. Those are necessary in auditory training especially in mapping for the young children. Comparison of the modifications of Stenver’s view and the classical interpretation of Stenver’s view were undertaken to document the insertion depth and numbers of intracochlear electrodes among the 11 cochlear implant cases in Chi-Mei Foundation Hospital between 1990 to 1994. There is a strong correlation between mapping results and plane X-ray films. The clinical utility of plane X-ray films in cochlear implant patients is well defined.
