摘要

自1988年7月至1996年6月，8年間因耳痛、耳部水疱或結痂、聽力障礙、耳鳴、顏面神經麻痺等症狀，至成大醫院耳鼻喉科就醫，經診斷為耳炎狀疱疹者，共29人;男性12名，女性17名。年齡分布為18到81歲，平均年齡為50歲，而50歲以上的患者就有17名。其中有6名病患因長期罹患其他疾病而身體狀況較差。臨床症狀以耳痛(100%)和耳部水疱(90%)最常見，而以聽力障礙(52%)、耳鳴(41%)、眩暈(38%)來表現也不少。腦神經的受侵犯以第七對(83%)和第八對(72%)為主，但顏面神經麻痺與水疱的發生則沒有固定的先後順序。所有病人都接受Acyclovir或加上類固醇治療，並沒有疱疹後神經痛的病例，但僅一半病人顏面神經神經麻痺的症狀獲得改善。本研究顯示耳帶狀疱疹仍以高齡或免疫力差的人較易罹患，治療上以Acyclovir和類固醇為主，對於減輕疼痛和減少後遺症都有不錯的效果，但顏面神經麻痺之復原仍有待突破。(中耳醫誌 1997;32:379-385)

Herpes zoster oticus, cused by Varicella-Zoster virus, is one of the causes of facial palsy. The appearance of a varicelliform rash, facial palsy, and otalgia are paramount in its diagnosis. A restrospective study of 29 patients with herpes zoster oticus that received therapy at our department from July 1988 to June 1996 was carried out. The clinical symptoms, disease course, treatment and prognosis were discussed. The mean age of these patients was 50 years old. Besides, six of these patients were debilitated due to chronic systemic diseases. Otalgia and auricular vesicles were the first and second most commonly reported symptom in these patients. The seventh and eighth pairs of cranial nerve are more apt to be invaded. There was no certain sequence in the onset of vesicles and facial palsy. After the therapy with acyclovir and steroid, most of the symptoms were ameliorated. No postherpetic neuralgia was noted during follow up. Unsatisfactorily, the recovery rate of facial palsy was only noted in half of cases presented. From this study, a poorer prognosis could be predicated when facial plasy is the first onset symptom and sign.
