以翻轉脂肪筋膜皮瓣重建上肢組織缺損

Adipofasical Turnover Flap for Resurfacing the Defect of Upper Extremities

摘要


翻轉脂肪筋膜皮瓣為一局部皮瓣,它具有簡單、有效、供應區不需補皮,避免主要血管傷害,保持較完整外觀及提供韌帶滑動界面的優點。經由前人對於前臂血行及穿通枝之研究,發現前臂具有豐富之血源供應,並經由多重之血管吻合,尤其在縱向上,使此皮瓣具有軸性皮瓣之性質,故能在皮瓣設計上,獲得較穩定之血行供應。自1990年3月至1992年12月,我們就手或腕部的軟組織缺損,使用此皮瓣成功地重建了10個病例。其受傷原因以電擊傷害居多,且併有重要血管、神經、韌帶等暴露,對於同一患肢有多處傷口時,使用脂肪筋膜皮瓣可保存主要血管,進而利用該血管供其他重建皮瓣之應用。基於臨床經驗,我們建議在考量上肢遠端軟組織缺損的重建時,使用此翻轉脂肪筋膜皮瓣是值得採用的方法。

Adipofasical turnover flap is a local flap, it has many advantages such as simplicity, reliability, lesser problem of donor site, no damage of main vessels, preservation of the natural contour and provision of the smooth surface for tendon gliding. This flap is used usually in movable part of body, such as hand and wrist. The axial pattern of flap is created by raising the flap including the prerforating artery and subcutaneous artery along the longitudinal orientation of the vessels and their interconnections. From March 1990 to December 1992, we successfully resurfaced the defect of upper extremities with adipofascial turnover flaps in 10 patients. Electrical injury is the most common etiology. The defect alway combined vessels, tendon and bone exposure. Since elevation of the flap does not sacrifice the main vessels, free flap can be performed in the same extremities concomitantly. The result is satisfactory, we recommend the adipofascial turnover flap should be considered in the management of the defect of upper extremities.
