摘要


外側上髁皮瓣為一軸性皮瓣,可提供作為足部軟組織的重建,但其缺點為供皮區往往必須植皮,影響美觀。 局部脂肪筋膜皮瓣主要是為來自基部的隨機性血循供應,已成功地被應用於肢體及軀體軟組織缺損之重建,但其存活受到皮瓣的長寬比値或基部與皮瓣面積比値的限制。我們將脂肪筋膜皮瓣的原理應用在外側上髁皮瓣,將其改良成帶有血管供應的脂肪筋膜皮瓣,如此可以不受長寬比的限制,手術可一次完成,且改善了原來外側上髁皮瓣供皮區必須植皮的美觀問題。此一原理似乎可被進一步廣泛應用到其他已知的帶血管莖皮瓣。

The lateral supramalleolar flap, supplied by the perforating branch of the peroneal artery, is versatile and reliable in reconstruction of soft tissue defects around the ankle and heel. The main drawback is donor site depression deformity. The local adipofascial flap, based on a random type of vascularity, was successfully used in reconstrcution of the defects in extremities and trunk. But the random flap has its limitation by length-to-breadth ratio of 2:1, or the flap-to-base area ratio of 4:1. We refined the lateral supramalleolar flap as an ”axial” adipofascial flap for three defects around the ankle. It has not only less donor site deformity, but also is not governed by the length-to-breadth ratio as is the conventional adipofascial flap. Furthermore, we preserved the superficial peroneal nerve in all cases, and all the patients had no sensory disturbance.
