Clinical Experience of Auricular Reconstruction with Retroauricular Myocutaneous Island Flap

應用耳後肌皮島狀皮瓣重建耳朵缺損的臨床經驗

摘要

耳朶舟狀窩或耳殼缺損的重建，對外科醫師來講是一種挑戰，特別是缺損等於或大於2公分時。此種缺損的重建之前曾使用，自然癒合、分層或全層皮膚移植及局部皮瓣；但是癒合時間較長、移動的限制及不能令人滿意的外觀經常無法避免。我們使用耳後肌皮島狀皮瓣重建一個惡性及三個良性腫瘤所造成的耳朶缺損，所有的皮瓣不僅完全地存活，並且獲致滿意的外觀。此種皮瓣，不僅有理想的顏色及質地、穩定的血流，而且可以一次手術及隱藏供給區疤痕。因此，對於耳朶缺損的重建，耳後肌皮島狀皮瓣不僅是一種選擇，而且是一種安全及美觀的皮瓣。

Reconstruction for the scaphal or conchal defects of the auricle is a challenge to surgeons, especially when these defects are 2 cm in size or larger. In this presented series, the retroauricular myocutaneous island flap was used to reconstruct four auricular defects which had resulted from resection of the tumors including squamous cell carcinoma in one scaphahelix and seborrheic keratosis in three concha. The size of the designed flap ranged from 3.8×1.5 cm to 4.0×1.5 cm. All flaps survived completely, and satisfactory auricular contour and hidden donor scar were provided postoperatively. The retroauricular myocutaneous flap receives blood supply from the posterior auricular artery and has good viability. It has advantages of ideal color match, good consistency and projection, one-stage procedure, and hidden donor scar. This is a good alternative for reconstruction of auricular soft tissue defects.
