摘要


隱孢子蟲症係一種感染於消化道與呼吸道的人畜共同傳染病。人類感染的第一個文獻病例報告於1976年,自此關於人類感染的病例日愈增加,尤其是後天免疫缺乏症候羣的病人尤易感染,更增加這種疾病的重要性。 根據本所1985年965隻豬解剖病例中,以組織病理學,穿透式與掃描式電子顯微鏡確診10隻豬患有腸道隱孢子蟲感染。這些感染豬均由於生前併發其他疾病而自然死亡或淘汰撲殺,其中6隻會有下痢病史。光學顯微鏡下。隱孢子蟲寄生於小腸絨毛的微絨毛及粘膜腺窩,出現的部位則以小腸後段的廻腸部分為主。組織病理變化主要為腺窩增生,絨毛上皮細胞變扁平、立方狀或壞死,以及部分絨毛萎縮與絨毛癒合,粘膜層則有大量單核細胞,嗜酸性球及少量嗜中性球浸潤。透過電子顯微鏡觀察,可以發現不同生活史中的隱孢子蟲。 目前,隱孢子蟲在本省已有感染於人、豬與家禽的病例報告。基於公共衛生醫學的觀點,極應對這種新的疾病有一概括性的認識。

Ten pigs with intestinal cryptosporidiosis from 965 necropsy cases were reviewed. Histopathology, transmission electron microscopy (TEA) and scanning electron microscopy (SEM) were used to identify the protozoan parasite and to confirm the diagnosis. All the pigs were complicated with other diseases prior to death or cull. Six of them had a history of diarrhea. Microscopically, Cryptosporidium sp. organisms were present on the microvilli of villous epithelium and/or within the mucosal crypts throughout the small intestine, particularly prominent in the ileum. Histopathologically, crypt hyperplasia, villous atrophy, villous fusion and epithelial damage with mononuclear cells, eosinophils and a few neutronphils were seen in the lamina propria of the mucosa in some cases. In addition, the organism was observed at different stages through TEM and SEM methods. In Taiwan, cryptosporidiosis has been found in humans, pigs and chickens recently. The literature dealing with cryptosporidiosis in all three species is also discussed in this article.
