A Sequential Change of Adenoma-Carcinoma in a Patient with Colonic Polyposis: Report of a Case
大腸息肉症腺腫與癌症之演變：一例報告
摘要
我們敘述一個腺腫與癌症演變的現象，並同時發生在一個大腸息肉症的病人身上。有趣的是：除了形態學和病理學上發現這些腺腫有從良性的息肉續漸變成惡性腺癌的趨勢以外，並且有從升結腸，橫結腸，降結腸到乙狀直腸依小到大的順序排列而分佈。所以這個病例，可能提供了有關腺腫與癌症演變研究的一個重要線索。 我們利用簡單的alcian-blue生化染色法作大腸的病理學檢查，不但可以作病理切片篩檢，並可用為診斷惡性變化和進一步研究的另一個簡單方法。 有關大腸直腸癌的防治方面，這一個病例更提醒臨床醫師們，一旦發現乙直腸病變，應該進一步作全大腸檢查，否則會錯失診斷了一些合併的惡性病變，特別是一些大腸息肉症的病例。因為腺性息肉與腺癌，是一個不可分割的問題。

We describe an event of sequential change of adenoma-carcinoma in a patient with polyposis of the colon. In which, the interesting arrangement of small adenomatous polyp, larger adenoma and adenocarcinoma from proximal colon to distal colon was presented. Based on the morphologic and pathologic changes from benign adenoma to adenocarcinoma, which were stained by alcian-blue histochemical method, the malignant sequence of adenoma-carcinoma seems to be under a condition of gradual transition of colonic mucosa from the ascending colon and descending colon down to sigmoid and rectum.
