頜下腺與導管間結石的改良手術方法
Improved Surgical Method for Removing the Sialolithiasis between the Submandibular Gland and Wharton's Duct•A Case Report
摘要
頜下腺導管內的結石一般經由口內法可以完全摘除，而位於導管深部尤其是位於腺體與導管移行部(hilum)的結石經由口腔內摘除是不容易達成，過去必須經由口外法，一併將腺體摘除，此往往造成病患日後唾液分泌減少現象，本篇報告兩個病例，在全身麻醉下，經由口內法將位於hilum處的唾石順利取出，並將頜下腺保留，使病患避免唾液減少的現象出現。

The sialolith inside the Wharton's duct can be removed completely through intraoral method. Those, however, in the deeper part of Wharton's duct, especially between the gland and hilum, can not be removed easily through intra-oral method. In the past, these kinds of stones had to be removed including the submandibular gland under extra-oral method. This kind of surgery will eventually cause decrease of saliva of the patients. Here we reported two cases of deeper sialolith. They were located in the hilum of submandibular gland and removed smoothly by intraoral method. By this special method, we preserved the submandibular glands and keep normal salivary flow.
