利用骨鑿式上顎竇提升術進行自體齒移植手術―病例報告

自體齒移植在臨床上常用來取代已不可保留之牙齒或用以重建缺牙的區域。然而，若是接受自體齒移植之區域位於上顎後牙區，則可能受限於上顎竇之解剖構造，而使得自體齒移植手術無法施行。本病例報告提出以骨鑿式上顎竇提升術(osteotome sinus lift)應用於自體齒移植來進行上顎後牙區之重建。患者爲20歲男性，因左上第一大臼齒嚴重蛀牙需要拔除，而同側有一嚴重顎側異位(palatal displacement)之第二小臼齒，故以異位之第二小臼齒進行自體齒移植，但因上顎竇底部過低，傳統的自體齒移植手術會因牙根長度太長而受限，因此合併施行上顎竇提升術，來達成自體齒移植的目的。患者術後無感染或鼻竇問題出現，並已追蹤三年，自體移植齒相當穩定，牙周組織健康，且與鄰接牙及對咬牙關係良好。因此運用上顎竇提升術不但可擴展自體齒移植之適應症，而且花費較少，更可以迅速恢復咀嚼功能，如果應用時機恰當，可提供上顎後牙區之重建治療的另一選擇；回顧文獻尚未有相關之報告，因此提出此一病例報告，供日後其他口腔外科醫師作爲參考。

Autotransplantation of tooth is a reliable method with a good prognosis to substitute a hopeless tooth. The maxillary sinus creates an anatomical limitation for ideal tooth transplantation. We represent a case who was received the autotransplantation of tooth combined with sinus lift and had a satisfactory result. The patient was a 20 year-old male and had a severely decayed left maxillary first molar. Because the patient had a palatally malposed second premolar at the same side of the mouth, we performed the autotransplantation of tooth combined with sinus lift to restore the extracted area right after the first molar was pulled out. Three years after the operation, the transplanted tooth was healthy in periodontium with stable occlusion. Autogenous tooth ransplantation combined with sinus lift technique expands the indication of tooth transplantation, and restores the masticatory function fastly and economically. It is an alternative option to restore the missing teeth at maxillary posterior area. To our knowledge, there is no relative literature reported.
