以具空間維持再生膜閉合長期之口竇相通-病例報告及文獻回顧

Closure of Long-standing Oroantral Communications with Space-maintained Biodegradable Membrane-Cases Report and Literatures Review

摘要


臨床上,許多口腔手術或病理狀況會造成上顎竇穿通,導致顎骨組織缺損,形成口竇瘻管。重建此類缺損的方式可施行皮瓣修補及移植手術,然而在上顎後區的骨移植預後較不易掌控,黏膜軟組織生長也常因其下無完整的覆蓋骨骼支持或張力過大,而導致癒合失敗。經改良後的生物可吸收性再生膜,其具有良好的機械強度及操作便利性,合併黏膜皮瓣手術覆蓋於口竇較大之骨缺損區,可隔離口腔及鼻竇區,阻絕軟組織的生長及陷入,具有不錯的屏障功能與部分骨引導效果。本文提出於三軍總醫院口腔顎面外科,使用具空間維持之生物可吸收性再生膜來閉合長期口竇相通的病例報告,以作爲臨床經驗之分享。

The sequel of oral surgery or pathologic entities may cause perforation of the maxillary sinus leading to formation of an oroantral fistula and osseous defect. Flaps or grafts can be used to close the oroantral communications, however, the failure will occur if an attempted primary closure with excess tension or without supporting by intact bone. The new modified biodegradable membrane with better barrier function, mechanical strength and ease of handling, which close oroantral communications combined with mucoflaps to isolate the oral cavity from maxillary sinus, prevent soft tissue ingrowth into defect, and enhancing osseous healing. Here we presented our experience in two cases at Tri-Service General Hospital with space-maintained biodegradable membrane to close the long standing oroantral communications.
