High Prevalence of Pain in Oral Cancer Patients in Taiwan-A Case Series Study and Literature Review

口腔癌病人的疼痛盛行率-系列病例報告與文獻回顧

摘要


疼痛的症狀在癌症病人身上很常見,甚至於病人就是因為疼痛求診,才被診察出癌症。本篇研究目的,主要評估台灣的原發性口腔癌病人族群,與癌症相關的疼痛盛行率與危險因子。研究對象為本院2005年至2007年間,總計78個連續無間斷的原發性口腔癌病人,回顧他們的病歷並加以分析。結果發現,78個口腔癌病人中,50個人(64.1%) 腫瘤有疼痛的表現;疼痛症狀與人口統計學,或菸酒檳榔的使用習性沒有共同關連。另外,在癌症初期沒有疼痛感的口腔癌病人,其存活率較被診斷時,就有疼痛主訴的病人為高。因此,教導民眾認識口腔癌的表現,特別是口腔內感到疼痛的病灶,並應提高警覺,乃是重要的課題。

Pain is a common symptom for patients with cancer and is one of the initial symptoms leading to the diagnosis of cancer. The objectives of this study are to evaluate the prevalence and risk factors for pain associated with oral cancer in patients at initial presentation in Taiwan. During 2005 till 2007, there are 78 consecutive primary patients with oral cancer enrolled in this study. The patients' charts were reviewed for demographic data, the social habits and orofacial pain. There were 50 out of 78 patients (64.1 %) that had reported pain associated with oral tumors. The orofacial pain was not correlated with the demographic data and social habits. The survival rate was significantly betler in the non-pain group than patients with pain perceived from the Kaplan-Meier analysis. Therefore, it is imperative to educate and create awareness for the cardinal signs of oral cancer, particularly the pain symptoms of the mouth.
