According to the various subtypes of squamous cell carcinoma in the 2005 World Health Organization's Classification of Head and Neck Tumors, neuroendocrine differentiation has the most aggressive clinical behavior and least favorable prognosis. Our case involves a 31-year-old Taiwanese male suffering from left buccal neoplasm with ipsilateral enlarged lymph node. After performing a wide excision with modified radical neck dissection on August, 2012, the histological examination of the specimens exhibited squamous cell carcinoma with basaloid neuroendocrine differentiation and neck lymph node metastasis. The neuroendocrine differentiation was confirmed with immunohistochemical stains. The patient expired from uncontrolled recurrences and metastasis in the bone, lung and neck lymph nodes six months after the major surgery. Due to a lack of case reports in oral cavity and oropharyngeal region and no standard sequential treatment guidelines, further investigation of possible treatment options and poor prognosis should be reminded.
