不同下肢運動模式對軀幹肌電活動的影響

摘要


目前下肢肌力強化的訓練分爲閉鎖鏈式及開放鏈式,對於此兩種肌力訓練模式相關於軀幹肌肉活動的影響,則尚無研究報告。本研究之目的爲分析軀幹肌肉在二種運動中的作用型態,並比較肌肉活動程度的差異。本研究以二十三名下背痛患者及九名正常受測者爲研究對象,分別從事等速膝關節伸展(開放鏈式運動)及下肢推蹬運動(閉鎖鏈式運動),記錄運動中腹斜肌與背脊肌的肌電圖變化。結果發現正常者的作用肌(腹斜肌),在兩種肌力訓練模式的整個運動過程雖均有共同作用,但在運動中、後期二者活動程度有差異,拮抗肌(背脊肌)則在整個運動過程均有顯著差異:下背痛患者的作用肌,在兩種肌力訓練模式的運動過程的中、後期,其活動程度亦有差異,但拮抗肌只在中期的活動時才有顯著差異。在閉鎖鏈式訓練的整個運動過程中,不論拮抗肌或作用肌,都是正常者高於下背痛患者:但在開放鏈式訓練的整個運動過程中,下背痛患者的拮抗肌並未低於正常者。閉鎖鏈式運動時,軀幹肌肉活動的共同作用範圍,比開放鏈式運動大,應可在運動中提供較佳的軀幹肌群訓練,研究結果期能作爲下背痛預防與復健的參考。

It is expected that either open or closed-chain mode of strengthening exercise for lower extremities provides the chance to strengthen the trunk muscles by the way of co-contraction which is an alternative method for back rehabilitation. The purpose of this pilot study was to examine the influence of the different muscle strengthening modes of lower extremity on the muscular activity of the trunk. It is divided into two stages of this study: 1) it is to collect the electromyographic data of the trunk muscles including trunk flexor (external oblique) and trunk extensor (erector spinae) during open chain (isokinetic knee extension) exercise and closed chain (leg press) exercise from the group of volunteers (n=9) without any injury over the trunk within 6 months.; 2) with the same procedures it gathers the data from the subjects (n=23) complaining low back pain without surgical intervention. More activities of external oblique occurred with open kinetic chain exercise, and erector spinae significantly increased in closed kinetic chain activity (p<0.05). It is suggested to select proper kinetic chain exercise as strengthening tool for lumbar muscle.
