全身麻醉下全口牙科治療應用於血液幹細胞移植前之準備

Full Mouth Dental Management under General Anesthesia as an Application for Patient before Hematopoietic Stem Cell Transplantation

摘要


血液幹細胞移植hematopoietic stem cell transplantation (HSCT),歷經多年的發展,現在已廣泛應用於白血病等血液惡性腫瘤,與嚴重型再生不良性貧血的治療,並且為其他先天性遺傳疾病,如嚴重型地中海貧血,脂類或粘多醣儲存疾病等的重要治療方法。但為避免植入之血液幹細胞受到排斥,移植前須進行高劑量化學治療與全身放射線照射,卻因免疫抑制作用造成病人暴露在感染的風險中,因此移植前的牙科治療與口腔照護有其必要性,而目的著重在偵測、消除與防止一切可能的口腔感染源(sources of infection),避免病患在免疫系統功能低落的期間,發生嚴重的感染而威脅生命。但由於難以預測的配對等待,化學治療的急迫性與白血球的低落等,都讓適切的牙科治療充滿挑戰。全身麻醉下一次性的全口牙科治療兼其時間效率,無菌操作,良好的出血量控制等優點,可機動性配合移植療程,迅速控制口腔感染源,並提供高品質的牙科治療成果。

Hematopoietic stem cell transplantation (HSCT) was developed as an important treatment for leukemia and severe aplitic anemia for years. However, pre-HSCT dental management was indicated to detect, eliminate and prevent any oral-dental infection source, which might proceed to life-threat infection. Full mouth dental treatment under general anesthesia has advantages of safety efficiency, aseptic practice, well bleeding control and no needs for children's behavior management. It appears to be appropriate for pre-HSCT patients with compromised immune systems and limited time for dental care.
