氧化鋯牙科植體表面骨整合之觀察

Observation of the Osseointegration of Zirconia Implant

摘要


目前市面牙科植體的主流材料是鈦金屬,因其擁有良好的生物相容性和優異的骨整合作用。惟其亦具有美觀或材質上一些無法改善的缺陷。近幾年來,市面上出現以陶瓷為材料的牙科植體,其目的乃是希望解決鈦金屬基質為植體的缺點。本實驗的目的係以動物實驗模式,觀察國產氧化鋯牙科植體表面的骨整合現象,並以鈦金屬植體作為對照樣本進行比較,期能建立常規之氧化鋯植體標本處理及評估模式,亦藉以了解目前氧化鋯植體臨床應用潛能的評估。結果顯示在長達八周的觀察中,氧化鋯植體與鈦金屬具有相似之骨癒合模式,兩者有接近的生物相容性與骨整合效果,未具統計學上的顯著差異,惟氧化鋯植體在一週時,其骨整合表現方面似乎較不穩定。以目前的研究結果來看,本研究已建立常規針對氧化鋯標本處理及評估模式,發現氧化鋯植體有近似於鈦金屬植體的生物相容性,在較長的觀察期中,亦可顯示氧化鋯植體具足夠的骨整合。儘管如此,氧化鋯植體性質及標本處理技術方面尚有需改進之處,期待未來能有進一步之研發與改良。

Titanium is the mainstream of materials used in dental implant today. It shows good biocompatibility and ability of osseointegration. But there are still some drawbacks of titanium implant. That's why ceramic dental implants have been advocated and developed in recent years. Goal of this study is to establish protocol of specimen prepareation for zirconia implants and to evaluate the efficiacy of osseointegration of zirconia implants with beagle animal model. Titanium implants were used in controlled group for comparison. Results showed no significant difference between zirconia implants and titanium implants in clinical parameters. Histopathological anaylsis from both grinding and decalcifed sections indicated similar bone-healing pattern between zicornium and titanium implants. Osseointegration of the zirconia implants in early healing phase (1 week) was not quite predicable as compared to titanium implants. We thus concluded that zirconia implants have good biocompatibility and may owe good osseointegration in longer healing period. However, further investigation on the specimen prepartion of the zirconia implants is needed for development of ceramic implant in the future.
