Management of a Perforating Internal Resorptive Defect: A Case Report
牙齒內吸收穿孔之處理－病例報告
摘要
牙齒內吸收在臨床上無症狀，通常在放射線檢查中被檢驗出來。利用三氧礦化物修復與牙周組織的穿通是一種治療方式。此病例報告為左側上顎側門齒之內吸收造成牙齒穿孔，利用三氧礦化物修復此缺損。在根管治療當中，利用氫氧化鈣作為根管內藥物並覆蓋臨時填補物，之後利用馬來膠針將根管及內吸收之缺損封填起來。治療完後，在斷層掃描下發現顎側部分有穿孔，以手術方式將此穿孔以三氧礦化物修復。六個月後的追蹤檢查上，在臨床及牙科放射線檢查下皆有良好的預後。

Internal resorption is a clinically asymptomatic condition commonly detected by routine radi-ographic evaluation. The use of mineral trioxide aggregate (MTA) is a conservative approach to repair lesions with periodontal communication. This case report presents a perforating internal re-sorptive defect at the maxillary lateral incisor repaired by MTA. During the endodontic treatment, calcium hydroxide was placed as a temporary dressing. After this period, the root canal space was filled with gutta-percha. After the treatment, a perforation on the palatal side was observed by the cone-beam computed tomography (CBCT). Surgical treatment including removal of granulation tissue and MTA displacement was done. The clinical findings and periapical radiographs indicated success of treatment until 6 months of follow-up.
