Most of the mandibular first premolar has one canal, however, the prevalence rate of mandibular first premolar with more than two canals complicated configuration is not low (12.9-46%). Therefore, endodontic therapy for these teeth is more difficult. A successful nonsurgical endodontic therapy depends on understanding of the root and root canal morphology. The occurrence of documented cases of a mandibular first premolar with Sert and Bayirli's type IX root canal configuration is rare. This case report describes the endodontic therapy of a mandibular first premolar with type IX root canal configuration.
