The pulp chamber morphology, the number of root canals, and the root canal configuration of the primary teeth often exist variations, and hence, the clinicians should always remember deeply the internal morphology of teeth such as pulp chamber and root canals at all times to achieve good quality in endodontic treatment. Missed root canals during root canal therapy is a common cause of endodontic treatment failure and may lead to postoperative pain, recurrent inflammation, and even treatment failure. Proper magnification tools and accurate images can help clinicians reduce the likelihood of missed root canals and even treatment failure. This is a case report of "the mandibular primary second molar with five root canals" treated with pulpectomy, glass ionomer fillings, and stainless steel crown (SSC) restoration. 14 months after treatment, a follow-up appointment revealed a good treatment outcome.
