摘要

臨床上多顆缺牙的病人，常面臨僅存的牙齒因臨床上治療的技術或牙醫師認為殘根的根管治療是無效益的治療，往往只能選擇拔除牙齒，最終必須製作傳統全口假牙。而傳統的全口假牙最讓患者抱怨的問題就是假牙容易鬆脫，因此患者常常要擔心假牙會脫落，進而影響患者的社交活動與身心靈健康。本篇病例報告為91歲男性，上顎患有多顆蛀牙與牙周病，並有心臟疾病服用抗凝血藥物，原診所牙醫師診斷需拔除上顎所有牙齒並製作傳統全口假牙，患者希望能有較保守的治療方式而至本院求診，經作者建議利用安裝牙根附連體（attachment），再製作馬蹄形（palatal-less）之上顎覆蓋式全口義齒（overdenture）來重建患者咀嚼功能，經12個月追蹤，患者相當滿意此假牙，牙根附連體的狀況維持良好並穩定。

Patient who already had multiple missing or decayed teeth often had been told the situation that all the remaining teeth would be removed and then a complete denture was made. The major problem of the traditional complete denture is its poor retention and falling out of mouth easily. People were always afraid of denture looseness, and this condition may then influence their social intercourse, mental and body health. This article reports a case of a 91 year-old man who wished to have more conservative treatment. He then received a maxillary palatal-less overdenture which had two natural root-supported attachments underneath the denture. After 12 months follow-up, the patient was satisfied with the denture, and the condition of root attachments was good and stable.
