In autotransplantation, the third molar can be selected as a donor tooth to treat patients with missing tooth problems. In this case, a 27-year-old male suffered from discomfort at upper left posterior area. With clinical examination, left maxillary third molar was found with large amalgam filling combined with tooth fracture, biting pain and percussion pain were also observed. Left maxillary third molar was diagnosed as previously endodontically treated and split tooth. After discussion with patient, we used right maxillary third molar as a donor tooth for autotransplantation after extraction of left maxillary third molar. After autotransplantation and endodontic treatment of the donor tooth, fair healing was observed and no discomfort was noted for 2 years follow up.
