Intramuscular hemangioma (IMH) is usually found in skeletal muscles of the trunk or limbs. There is about 10 to 15% occurring in the head and neck region, usually involving the masseter muscle. We present a patient with IMH showing in the left buccal cheek. A healthy 33-year-old male was referred for the evaluation and management of a swelling, purplish-red buccal mass. Preoperative magnetic resonance imaging (MRI) showed a vascular tumor in the left cheek adjacent to the buccinator muscle. Angiography was applied to determine the tumor feeding vessel and preoperative transarterial embolization of the lesion facilitated subsequent surgical treatment by reducing intraoperative blood loss. Intraoral removal of the lesion was performed, and the patient recovered well after operation.
