Reduction Nippleplasty for Irregular Shape with Tri-flaps and Central Core Preservation-Case Report

摘要

背景：
乳頭是乳房美觀中一個很重要的關鍵，除了乳房本身的大小與形狀之外，乳頭的形狀也會深深的影響了整體乳房的外觀。
乳頭肥大是一個惱人且常常在亞洲婦女發生的問題。它有時是家族性的，且通常出現在青春期後，或是在懷孕之後，並且一直到更年期。
在過去的文獻中，已經有很多乳頭整形術的方法，但是這些方法並沒有辦法同時保留哺乳功能、感覺，且同時運用在不規則形狀的乳頭上。
目的及目標：
我們設計一個新的乳頭縮減整形術方法，使用三個皮瓣並保留中央圓柱，可以用於不規則形狀的乳頭，可以保留哺乳功能並同時保留乳頭的感覺。
材料及方法：
我們報告一位兩側乳頭不規則肥大的32歲女性，她在一年前接受過雙側隆乳手術，但仍對自己的乳頭不規則肥大感到困擾，因此尋求醫療上的協助。我們使用三個皮瓣並保留中央圓柱的方法，來矯正此病人的雙側乳頭不規則肥大。
結果：
我們成功的使用新的方法，來矯正此病人的雙側乳頭不規則肥大，術後傷口癒合良好，沒有皮膚壞死，且保留了乳頭感覺，病人對術後乳頭美觀感到滿意。
結論：
我們使用新的乳頭縮減整形術的方法，可以用於不規則形狀的乳頭，並同時保有乳頭的美觀與功能。

Background:
Nipple hypertrophy is a bothersome problem and frequently seen in Asian women. It is sometimes familial and usually appearing after the onset of adolescence or following pregnancy, and persists through menopause. Numerous methods of reduction nippleplasty have been described in literatures. However, neither of them can preserve the lactation, sensory and be applicable on irregular shape simultaneously.
Aim and Objectives:
Here we present a new method of reduction nippleplasty with tri-flaps and central core preservation. This method can preserve the lactation and nipple sensation, and can be used in irregular-shaped nipple as well.
Materials and Methods:
A 32 years old female, who had received bilateral augmentation mammaplasty 1 year ago, was bothered by bilateral irregular hypertrophic nipples. She asked for medical consultation. Reduction nippleplasty with tri-flaps and central core
preservation was used to correct her irregular hypertrophic nipples.
Result:
We corrected the irregular hypertrophic nipple of this patient with tri-flaps and central core preservation successfully. Postoperatively, the wound healed well without skin necrosis, and the sensation of the nipple was well preserved. This patient was satisfied with the aesthetic result.
Conclusion:
Our new method of reduction nippleplasty can be used in irregular type of nipples. With our method, both the aesthetic and functional integrity of the nipple are preserved.
