Serum Fibroblast Growth Factor 23 Levels in Taiwanese Patients with Stage 3 and 4 Chronic Kidney Disease

BACKGROUND: Serum fibroblast growth factor 23 (FGF23) levels was shown to be highly associated with the incidence of cardiovascular risk and left ventricular hypertrophy in patients with non-dialysis chronic kidney disease (CKD). This study tried to evaluate the clinical significance of serum FGF23 levels in CKD patients. METHODS: In this cross-sectional observational study, 53 stable Taiwanese stage 3 and 4 CKD patients were recruited. Serum FGF23, 25-hydroxyvitamin D, hemoglobin, serum electrolyte, HbA1c, intact parathyroid hormone, lipid and iron profiles, plasma renin and aldosterone levels, and estimated glomerular filtration rate (eGFR) were measured. RESULTS: Serum FGF23 levels showed inverse correlation with eGFR. In subgroup analysis, the presence of diabetes mellitus had significant effect on the inverse correlation between FGF23 and eGFR, even after multivariate adjustment. CONCLUSION: Serum FGF23 levels have inverse correlation with eGFR values in Taiwanese CKD stage 3 and 4 patients. Diabetes show significant effect on this inverse correlation.
