摘要
鼻及鼻竇的惡性畸胎上皮細胞肉瘤(teratocarcinosarcoma)是極為罕見但極度惡性的腫瘤，其組織學特色為同時包含惡性畸胎瘤，上皮細胞癌及惡性肉瘤。通常發生在成年人，平均54.7歲發生，以男性居多。本文報告1名55歲男性，因兩週內右側鼻孔反覆流鼻血及鼻塞求診，經視診及內視鏡檢視發現一鼻腔內腫瘤，無明顯頸部腫塊，安排核磁共振檢查顯示在右側鼻腔，篩竇，及後鼻孔有增強顯影疑似腫瘤之發現，並無明顯顱內侵犯之現象，雙側頸部並無明顯淋巴結之轉移。經內視鏡手術切除鼻及鼻竇之腫瘤，病理報告證實為惡性畸胎上皮細胞肉瘤，術後並安排化學治療合併放射治療。因病例罕見，特提出報告。

Sinonasal teratocarcinosarcoma is a rare malignancy in the sinonasal cavity. The histological morphology includes malignant teratoma, carcinoma and sarcoma. A 55- year-old man came to our emergency room due to the right side epistaxis. Endoscopic examination revealed right nasal granular tumor with touch bleeding. Computed tomography scan revealed soft tissue mass occupied in the nasal cavity, ethmoid sinus, maxillary sinus, frontal sinus, and sphenoid sinus. The nasal septum deviated to the left side due to tumor compression. MRI revealed enhanced mass in the right nasal cavity, ethmoid sinus, and choana without obvious intracranial invasion or neck lymph node metastasis. The patient received endoscopic surgical excision of nasal tumor. The histopathologic diagnosis was teratocarcinosarcoma. Post-operation concurrent chemoradiotherapy was arranged.
