探討精神衛生護理師核心護理工作表現狀況

摘要


護理實務能力是維持護理品質之必要條件。精神衛生護理師認證考試能檢定精神衛生護理師之護理實務能力。然而,於臨床情境直接觀察護理人員之工作表現也是持續評價護理師實務能力之重要方法。本研究旨在探討精神衛生護理師及其直屬護理主管對護理師於臨床照護情境中核心護理工作表現之評價。資料收集時間自97年2月至4月,共有71位精神衛生護理師進行自評及其直屬護理主管以實際觀察評價精神衛生護理師(69位)之核心護理工作表現,並以自填式問卷提供資料。運用描述性統計分析及獨立樣本t檢定,分析精神衛生護理師自評核心護理工作表現與其直屬護理主管評量結果之差異性。本研究中精神衛生護理師及其主管之工作年資均超過10年以上,他們一致反應80%-90%之精神衛生護理師臨床核心護理工作表現能達精熟(proficient)程度,兩者間評價結果並無顯著差異(t=-.66, p=.51),展現精神衛生護理師在臨床照護情境中確能提供優良的護理服務品質。另有10-20%精神衛生護理師有部分核心護理工作項目,仍需加強執行之精熟度,這些工作項目可為精神衛生護理師在職訓練之參考項目。

The competency and task performances of staff nurses in clinical settings are the central issues of improving qualities of care. Accreditation Systems of Psychiatric Mental Health Nurses (PMHNs) can effectively examine the clinical competences of PMHNs. Direct observation in the real situation is another measuring method to continue evaluates the clinical performances of PMHNs. The purpose of this study is to evaluate the clinical nursing performances of the PMHNs. The self-report structural questionnaires were used to explore the views of 71PMHNs and their supervisors (evaluated 69 PMHNs). Data were collected between February and April in 2008. The data were analyzed by using descriptive statistics and independent t-test. The clinical psychiatric nursing experiences of these subjects, including PMHNs and their supervisors, were all more than 10 years. Analytical results demonstrated that the clinical nursing performances of PMHNs has no significant difference (t=-.66, p=.51) in statistics, which was approved by the evaluations of the PMHNs and their supervisors. Additionally, 80%-90% of the PMHNs' clinical nursing performances could achieve the level of proficiency and high proficiency. However, 10-20% of PMHNs in parts of clinical nursing performance could not achieve fully proficient. The information shown above not only indicated that the PMHNs could provide clinically proficient care in a holistic manned, but also suggested that nursing department should apply actively the strategy of clinical training to promote the psychiatric nurses' competences and clinical performances.
