摘要

目前國內有關居家呼吸器依賴病患照護經驗甚少，本文為出院準備護理師以個案管理者的角色，運用出院準備服務的概念，透過觀察、會談、家庭評估等方法，評估個案與照顧者之需求，協助一位慢性阻塞性肺疾病引發呼吸器依賴的患者，在急性醫療過後，成功返家的經驗。在出院準備服務收案服務期間，除指導個案家屬學習相關照護技巧外，並召開家庭會議協助家屬重新調整家庭功能，及按個案需求轉介聯繫相關單位。在整體照護過程中，案家對出院準備服務個案管理者之功能表現肯定與感謝。筆者在後續個案與家屬問題追蹤時，發現照顧者體力與心理負荷，需依賴社會資源與政府相關單位重視，才能提供個案與家屬更完善之持續性照護。本研究期能提供臨床實務工作者，照護類似此個案之參考。

At present, there are very few experiences of assisting home-care ventilator-dependent patients in Taiwan. Form this article, a registered nurse, play as a case-manager, will discuss a successful experience through observation, discussion and home evaluation, from assisting a ventilator-dependent patient, who was suffered from COPD. It will help offering some similar experiences for those clinical workers. During the time of preparing those patients being discharged, not only she need to teach those family members to learn home-caring skills, but also she need to help re-adjusting functions of family members through a meeting, and refer them to the associated department whenever they needs help. The patient and his family were both approved the functions of preparation and show appreciation to the author after the patient discharged. The author of this article found that they need the help from related associations of social and government to lessen the physical and mental burden of the care-givers; thus, providing continuous and complete caring to the patients and the families would be our goal.
