本文為一研究對於廣泛性燒傷病人早期以鼻-十二指腸灌食(nasoduodenal feeding),其效果與安全性。比較早期鼻-十二指腸灌食和傳統性灌食,其熱量供給之差異,將病人分為鼻-十二指腸灌食及傳統性腸道灌食兩組。若考慮腸道灌食加靜脈營養,鼻-十二指腸灌食組(A組)病人燒傷後第三天其熱量供給達100.1±191%,傳統性灌食組(B組)則為63.8±21.3% (p=0.001)。若只考慮腸道灌食所供給之熱量,鼻-十二指腸灌食組病人燒傷後第三天其熱量供給達77.8±21.9%,傳統性灌食組則為37.7±16.7% (p=0.001)。此外,早期鼻-十二指腸灌食所引起之併發症很少。 研究結果顯示,對於廣泛性燒傷病人早期熱量供給之程度,鼻-十二指腸灌食確實優於傳統性灌食。本篇研究證實廣泛性燒傷病人早期以鼻-十二指腸灌食,對於熱量之供給確實是一有效且安全之方法。

This is a controlled prospective study about the safety and efficacy of early nasoduodenal (N-D) feeding for extensive burn patients. A comparison of caloric supplementation in-between early N-D and conventional enteral feeding was done. The results showed that early N-D feeding was superior to conventional enteral feeding. The overall, enteral plus parenteral, caloric supplementation reach to 100.1±19.1% on the 3(superscript rd) postburn day in N-D feeding group and only 63.8±21.3% in enteral feeding group (p=0.0001). Considered the enteral supplementation only, the N-D group was also superior to conventional enteral feeding group, 77.8±21.9% versus 37.7±16.7% (p=0.0001) on the postburn day. The complications of early N-D feeding were minimal. This study proves that the early N-D feeding is a safe and effective method to supply nutrition for the extensive burn patients.
