  • 腳踝扭傷為常見運動傷害之一,腳踝扭傷常發生在需要大量反覆跳躍著地的情境當中。根據研究統計,腳踝扭傷中有80%為內翻扭傷,這些經歷過腳踝扭傷的患者中,可能因為受傷導致腳踝神經肌肉損傷、本體感覺缺失與運動控制的能力下降,而使得扭傷容易再次發生,最後形成慢性腳踝不穩定 (Chronic Ankle Instability, CAI)。過去對於慢性腳踝不穩定之研究,大多在比較健康人與不穩定族群之差異,然而健康人在過去未曾有過腳踝扭傷,因此在某些程度上無法推測不穩定組之特徵。為此,本研究加入曾經經歷過一次腳踝扭傷病史實驗參與者 (Copers),配合不同高度著地反跳動作,希望找出三個族群間 (健康組、潛在組與不穩定組) 有何漸進關係與特徵,藉以作為降低及預防腳踝重複扭傷之風險。目的: 探討不同踝關節形態從不同高度著地反跳的影響。方法: 尋找29名大專甲組運動員分別區分為健康組、潛在組與不穩定組。實驗工作進行不同高度 (10公分、30公分、50公分) 著地反跳測驗;利用十台Vicon紅外線攝影機 (200Hz) 與兩塊Kistler測力板 (1000Hz) 同步收集下肢生物力學參數;使用二因子混合設計比較高度與組別間有無交互作用和三組間有無差異。結果: 不穩定組在著地瞬間的踝關節外翻角度高於健康組(p < .05),不穩定組在下蹲最低點踝關節外翻高於健康組與潛在組 (p < .05)。結論: 不穩定組踝關節著地階段的外翻動作可能為預防再次扭傷代償動作,有穩定關節與降低踝關節內翻風險之作用,潛在族群在動作過程中會表現出與其他兩不同的動作模式,健康族群踝關節背屈力矩顯著大於不穩定族群(p < .05),歸因於受傷後背屈肌群控制不佳所致,且有過扭傷病史族群會減少足底壓力中心(COP)左右擺盪範圍和面積來穩定平衡,本研究發現踝關節不穩定族群在執行著地反跳時會採取不同動作策略,並且利用更複雜策略來維持關節穩定,而在三組間確實存在依序變化的趨勢和特徵。

    The purpose of this study was to investigate the effects of different height of box drop vertical jump (DVJ) among the healthy control, coper, and CAI groups. Participants were asked to perform a box drop vertical jump and followed with a single-leg landing on the force plate. The DVJ height was 10cm, 30cm, and 50cm. Results from the current study indicated our major findings: CAI groups showed in frontal plan when initial contact, In sequence for CAI ,coper and healthy group,and Copers showed mores knee flexion and hip flexion when initial contact,and more eversion torque when initial leave the ground, CAI ,Copers groups decrease the COP(mediolateral and area range),this may be a mechanism to prevent reinjury.
