Squamous Cell Carcinoma of the Tonsillar Region a 13 Year Experience

扁桃腺上皮細胞癌十三年的臨床經驗

摘要

統計自民國64年1月至民國75年12月，榮民總醫院癌病登記電腦裡的六十九例扁桃腺上皮癌病例。18%的病人為第一和第二期患者，81%為第三和第四期患者。71%的病人接受放射线治療，10.2%攘受化學治療和放射棧治療，7.2%接受手術以及11.6%的病人接受手術和放射线治療。放射线治療對於T1、T2、T3、T4的病灶分別能達到100%, 90.9%, 89.4%及41.7%的局部控制率，然而復發率亦分別為14.3%, 20%, 29.4%和80%。三年真實存活率第一和第二期病人為70%，第三期為58%，第四期為40%。對於T1，T2的早期病灶，放射线治療和手術治療有相等的療效。

Sixty-nine patients with squamous cell carcinoma of the tonsillar region were treated and analysed from January 1975 to Decemer 1986 Ninteen percent (13/69) of patients presented with stage Ⅰ and Ⅱ disease, 81% (56/69) with stage Ⅲ and Ⅳ. Seventy-one percent (49/69) of the cases were treated by radiotherapy alone, 10.2% (7/69) by neoadjuvant chemotherapy and radiotherapy, 7.2% (5/69) by surgery alone and 11.6% (8/69) by surgery and radiotherapy. Initial local control rates of 100% of T1, 90.9% of T2, 89.4% of T3, and 41.7% of T4 lesions were achieved by radiotherapy alone, and the local relapse rates were 14.3%, 20%, 29.4% and 80% respectively. Three year actuarial survival rats were 70% for stage Ⅰ and Ⅱ patientes, 58% for stage Ⅲ and 40% for stage Ⅳ patients. Radiotherapy alone was as effecive as surgery in Ti and T2 tonsillar carcinoma.
