Malignant Fibrous Histiocytoma of the Parotid Gland

惡性纖維狀組織細胞瘤（Malignant fibrous histiocytoma）好發於四肢或後腹腔，僅約3％發生於頭頸部，而原發於腮腺內更是罕見。本文報告1名82歲女性因右側耳下方有一腫塊已經半年多，在近1個月內腫塊快速增大到8×7公分大，但未有顏面神經麻痺或是皮膚侵犯之表徵。電腦斷層掃描顯示在右側腮腺有一多分隔、內含水囊之腫塊。經右側腮腺全葉切除手術，組織病理診斷為惡性纖維狀組織細胞瘤。此腫瘤惡性度及轉移機率皆很高，該病患術後有接受進一步放射治療，但仍於術後6個月因局部復發和遠端轉移過世。查閱文獻惡性纖維狀組織瘤原發於腮腺內僅有22個病例報告，因屬罕見，特別提出供大家參考指教。

Malignant fibrous histiocytoma (MFH) occurs most commonly in the extremities, followed by the retroperitoneum. Only approximately 3% of MFH are found in the area of the head and neck and they are rarely seen in the parotid glands. This article presents an 82-year-old female whose chief complaint was a progressive mass on her right infraauricular area for 6 months. During the recent 1 month, the mass rapidly enlarged to 8 × 7 cm without skin ulceration or facial palsy. Computed tomography showed one large mutiloculated tumor with cystic component in the right parotid gland. Right total parotidectomy was performed. The histology revealed a storiform-pleomorphic type of MFH with high mitotic rate. Although postoperative radiation therapy was performed, the patient died of local recurrence and distant metastasis at post-operative 6 months. Our review of the literature yielded only 22 reported cases of MFH arising in parotid. We would like to put this case forward and discuss its diagnosis and treatment.
