高度近視併原發性隅角閉鎖性青光眼－病例報告
Primary Angle Closure Glaucoma in High Myopic Eyes-A Case Report
摘要
本文提出一例兩眼高度近視併有原發性隅角閉鎖性青光眼之病人，病人眼壓、視野、視神經盤凹陷及隅角鏡檢查約顯示為隅角閉鎖性青光眼，而其兩眼眼軸長度分別為右眼28.09mm；左眼27.38mm，兩眼屈光為右眼－18.0/-2.50×155；左眼－12.0/-0.50×30。雖然高度近視眼在解剖學之特徵被認為可保護避免隅角閉鎖性青光眼之發作，然高度近視患眼並不代表就能免於此病。病人在接受雷射虹彩造孔術(Laser Iridotomy)及隅角整形術(Gonioplasty)後，目前以0.5% Betoptic(上標 ®)一天兩次點藥可將眼壓控制在12~18mmHg之間。

We reported a 53 years old female patient, who suffered from primary angle closure glaucoma. Ocular examination revealed elevated intraocular pressure, visual field defet and enlarged optic disc cupping. Gonioscopic examination revealed peripheral anterior synechiae over the upper, nasal and lower quadrants. A-scan biometry showed the axial length OD 28.09mm; OS 27.38mm. VA with correction revealed OD 0.03 (0.2x-18.0/-2.5x155); OS 0.05 (0.2x-12.0/-0.5x30). Argon laser iridotomy and gonioplasty were performed. After the laser treatment the IOP was controlled around 12-18 mmHg with 0.5% Betoptic twice a day till now. Myopia is associated anatomical factors that offered considerable protection from primary angle closure glaucoma, its presence does not eliminate the possibility of the disease.
