應用參數分析探討齒顎矯正微植體之生物力學效應

摘要


穩定的微植體提供了良好的矯正錨定,對齒顎矯正治療有著重大的影響。本研究以電腦模擬下顎骨後牙區之解剖構造,並於小臼齒與大臼齒牙根間之置入微植體,以有限元素法針對皮質骨厚度、微植體外徑及施力角度進行參數分析,探討其生物力學效應。結果顯示微植體外徑會影響皮質骨應力分布情形。皮質骨厚度、微植體外徑和施力角度皆對海綿骨有明顯影響。本研究所選擇之三個參數與統計上皆對小臼齒與大臼齒有影響,但其應力值遠小於其他部位。施力角度與微植體外徑明顯影響微植體分擔矯正應力之能力。

A stable mini-implant provides a splendid orthodontic anchorage, which leads a profound influence to the treatment course. This study used finite element method, with a molar and premolar on the posterior mandible by computerized simulation, to investigate the biomechanical effects of the orthodontic mini-implant. Factorial analyses were performed including the cortical bone thickness, the diameter of the mini-implant and angle of the orthodontic force. The results showed that the diameter of the mini-implant affected the stress distribution on the cortical bone. All parameters had significantly effects on the cancellous bone stresses. The force angle and the diameter of the mini-implant altered the ability of the mini-implant which carries the orthodontic stresses.
