摘要

接受全價關節置換術人數逐年增加，近年來陸續有研究運用健康指導提昇手術成效，但卻缺乏系統性文獻回顧探討本土性之健康指導措施介入成效。本文目的為透過系統性文獻回顧探討全價關節置換手術健康指導措施成效。鍵入「hip replacement」、「health education」、「effectiveness」等關鍵字，搜尋1966年至2012年，CINAHL、Academic Search Complete、Medline、Cochrane Library及CEPS所發表的文獻。由兩位獨立研究者，以澳洲實證中心發展之量，性研究評讀工真評讀文獻品質。研究結果共收錄6篇文獻，5篇為隨機控制研究，1篇為類實驗研究。文獻品質得分介於9~11分。研究結果顯示給予全價關節置換手術健康指導措施的實驗組個案住院天數較對照組為少，且實驗組個案三個月內身體活動功能較對照組為佳。健康指導介入措施對全價關節置換手術個案不但可減少個案住院天數，且可促進個案身體活動功能，有助於個案生活品質的提昇。

Annually, the number of patients undergoing total hip replacement increases. Recently, medical research has successively determined that the application of health navigation programs promoted operative effectiveness. However, there is a lack of systemic review of domestic health navigation programs that aim to improve operative effectiveness. This systematic review probes the effectiveness of health navigation programs for patients undergoing total hip replacement. Keywords, such as ＂hip replacement＂, ＂Health education＂, ＂ffectiveness＂ were included in a research article search. Common databases, such as CINAHL, Academic Search Complete, Medline, Cochrane Library of Controlled Trials, CEPS, etc. were used to search for published articles between 1960 and 2012. Two independent researchers participated in this review. The quantitative appraisal instruments, developed by Australian Evidence Center, were used to assess the quality of the research articles. Six studies were reviewed with five randomized controlled trials (RCTs) and one quasi-experimental design study. The assessment scores of these studies ranged from 9 to 11. The review outcomes indicated that the experimental group with health navigation had fewer hospital days and better mobility function in three months post-operation than the control group. The study concluded that health navigation programs can reduce the number of hospital days, promote patient mobility, and facilitate better quality of life for patients undergoing total hip replacement.
