協助使用呼吸器之肺癌病患面對焦慮-Watson's關懷理論之運用

The Application of Watson's Caring Theory: Helping a Lung Cancer Patient to Face Anxiety on a Ventilator

摘要


肺癌是一種高致死率的疾病,對病患身、心、靈皆造成極大的打擊;而呼吸器之使用,更使病患因為侵入性的治療及疾病的威脅,產生生理與心理的壓力,病患在這些多重的身、心、靈之壓力下常會導致焦慮、害怕、焦躁的情緒反應。本文主要是探討一位呼吸器使用之肺癌末期病患於加護病房住院期間,面臨疾病與死亡威脅下所產生之焦慮反應。筆者藉由連續性的參與照顧,利用觀察個案語言及非語言的行為及與家屬溝通中,將個案的焦慮之導因進行分析、歸納及整理;並應用Watson's關懷照顧理論中的十項關懷照顧因素,在個案及家屬互信的基礎下,以促進個案生理的舒適、鼓勵情緒的表達、提供照護資訊等方式來緩解肺癌末期使用呼吸器個案之焦慮。

Lung cancer carries a high death rate and usually comes as tragic physical and emotional shock to the patient. Because of invasive treatments and threatening illness, patients experience increased physical and mental stress during the time that they are using the ventilator. Under such circumstances, lung cancer patients often feel very anxious. This paper explores the reflections on anxiety of an end-stage lung cancer patient on a ventilator during his stay in an intensive care unit as he faced increased illness and impending death. While caring for the patient, we collected and analyzed the sources of the patient's anxiety by observing his verbal and nonverbal behavior and observing his communication with his family. Using Watson's caring theory, we gained the trust of the patient and members of his family, reduced his physical pain, encouraged him to express his emotions, and offered the family information to reduce anxiety of the end stage of a lung cancer patient.
