A 22-year-old male military recruit suffered discomfort in both hips and the groin area for 2 months. He had no history of major trauma. Initial radiographs and bone scan demonstrated fracture of all 4 public rami. He received conservative therapy, including activity restriction and pain relief. Three months later, he was symptom-free and able to resume previous training activities. We present a male patient with straddle-type pelvic stress fracture and discuss the possible causative mechanisms.
