一位食用減肥菜致氣切患者之護理經驗

本文乃是協助一位食用減肥菜致呼吸衰竭婦女，在經歷身體痛苦外，緊接著又面對氣切需終日依賴呼吸器所帶來的心理適應過程。於91年2月21日至91年8月21日的照護期間，藉由觀察、會談等方式，在某區域教學醫院呼吸照護病房中完成資料收集。本文運用生理功能、自我概念、角色功能、相互依賴等羅氏適應模式為依據，以第二層次三種刺激評估，終確立個案的主要護理問題為無力感、低自尊、角色扮演改變、言辭溝通障礙。針對影響個案的適應程度，以全家為工作單位，提供愛與關切的情緒支援，對失落的心情表達同理心，肯定存在的價值及自我尊嚴，並提供有關病情的資訊支援，使個案體認疾病為不可逆的阻塞性肺疾病，學習與疾病共存，加強自我照顧，幫助病人適應疾病所帶來的生活改變。雖然只是一例護理照顧經驗，透過不斷的觀察評估與回饋，可以感受到羅氏適應模式在評估慢性病患所帶來的價值，盼能提供護理人員照顧病患時之參考。

This case study focuses on the experience of providing nursing care to a female patient who suffers from physical and emotional pain subsequent to respiratory damage and tracheotomy following Suaropus andrgynus intoxication. Clinical observation and interview data were gathered for 6 months at the respiratory care unit of a regional teaching hospital. The Roy Adaptation Model (physiologic-physical function, self-concept identity, role function, and interdependence. etc.) was the conceptual framework used in this case study. The nursing process. problem-solving approach was used to aggregate data and stimuli were classified in order to evaluate the patient. The major obstacles to providing nursing care for this patient were identified as emanating from her feelings of powerlessness, low self-esteem, functional role change within the family, and conmunication problems. To manage the patient's adaptation to negative stimuli, her family members were encouraged to promote interdependence as a family unit. The family served as a resource in providing care, love, understanding, and support. The family’s involvement helped the patient to not feel isolated and alone in dealing with her condition. Furthermore, they created a positive attitude of respect and hopefulness that the patient can learn how to cope with the fact that she has chronic, obstructive pulmonary disease. The experience of using the Roy Adaptation Model in determining nursing care for this patient is offered as an example for other nurses facing the difficult charge of providing nursing care for a patient with an irreversible condition.
