本文描述一位腰椎損傷引發神經性膀胱婦女,協助適應日後須每日行間歇性導尿之護理經驗。筆者於2018年12月5日至12月22日,透過評估及收集資料確立個案有排尿型態障礙、照顧者角色緊張、無力感之護理問題。筆者照護期間運用女性生殖器模具衛教個案間歇性導尿注意事項及技巧,減輕個案焦慮並能勝任執行技術,進而協助個案重建排尿型態。另外,也適時給予照顧者鼓勵和紓壓方式以緩解緊張之情緒,增進其協助個案執行間歇性導尿之信心,故藉由傾聽、同理、陪伴及關懷方式,加上家人間支持及鼓勵,使個案接納現況,願意再次面對挑戰,重拾日常生活。期望藉此經驗分享,以提供日後護理同仁之參考。
This article describes a nursing experience in helping a woman with traumatic spinal cord injury-induced neurogenic bladder adapt to intermittent self-catheterization in her daily life. From December 5^(th) to December 22^(nd), 2018, assessment was applied to evaluate and collect data. The authors were able to establish the patient had three nursing issues-impaired urinary elimination, caregiver role strain, and powerlessness. During the nursing period, an anatomical model of the female genital organs was used as a teaching aid to help the patient learn the techniques and precautions of self-catheterization, reduce her anxiety, and enable her to be competent in the task, in order to restore normal urinary elimination pattern. Meanwhile, the authors also gave the caregiver encouragement and provided ways to relieve his stress and nervousness, to make him more confident in helping the patient perform intermittent self-catheterization. By careful listening, empathetic attitude, companionship and care initiatives plus the family support and encouragement, the authors were able to motivate the patient to accept the situation, make her willing to overcome the challenge again, and ultimately help her regain her everyday life. With the sharing of this nursing experience, the authors hope to provide a clinical reference for other nursing staff.
