目的:本文敘述臨床疼痛護理師協助一位過去因不良止痛經驗患者於此次的疼痛照護經驗,護理期間自2009年4月30日至5月4日。方法:藉由持續性關懷、訪談、傾聽、觀察、身體評估及查閱病歷等方法收集資料,並應用Gordon十一項健康型態進行評估,確立護理問題有:焦慮手術風險/術後傷口疼痛控制的成效有關、急性疼痛/手術後傷口有關、特定知識缺乏/缺乏高血壓照顧資訊有關。結果:於術前提供相關疼痛護理處置,緩解手術後傷口疼痛。結論:手術前提供個案及家屬有關疾病及手術後疼痛處理的資訊,以增強個案對疼痛護理的認知及因應措施,緩解其焦慮;手術後協助個案使用病患自控式止痛,減緩傷口疼痛不適,改善睡眠品質,使個案獲得持續性和個別性的護理照顧。
Objective: This case report describes nursing experience while helping a patient deal with anticipated pain and anxiety related to an operation. This patient had been frightened by previous surgery and fear the surgery needed for recovery due to the previous pain. The nursing care lasted from April 30 to May 4, 2008. Methods: Data were collected by bidirectional communication with the patient, accompanying the patient, listening to the patient, observing the patient, by physical assessment and by chart-reading. Results: The results were analyzed using the Gordon's 11-item health assessment. The nursing problems were identified as: (1) anxiety about and a poor perception of the pain associated with past surgery, (2) acute pain associated with the tissue (operation) wound and (3) a specific knowledge deficit in the area of hypertension care. The results indicated that the nursing management that was provided before operation was able to successfully relieve the pain from operation and had also helped the patient to regain her confidence. Conclusions: The patient and her family developed greater confidence in relation to the patient's illness. The hospital provided knowledge that supported the patient as well as information she could understand, which reduced the patient's level of panic. Nursing care was continuously provided until she had recovered. In conclusion, this was a successful intervention in that the nursing care provided helped the patient to improve her condition both physically and mentally.
