小柴胡湯改善耳鳴及聽力障礙

一位45歲女性，左耳耳鳴已5年，1年前左耳聽力開始減弱，在3個月內左耳聽力從27分貝、53分貝、67分貝、82分貝、87分貝，持續惡化至92分貝，經X光、腦波、核磁共振檢查，未發現異常現象。患者因服西藥後會有心悸、胃腸不舒服等現象，而且左耳聽力仍持續惡化，半年前乃停止服用西藥，至中醫門診就醫。本個案經以小柴胡湯為主方治療3個月後，聽力開始從92分貝減為88分貝、80分貝，聽力損失由極重度，改善為重度。耳鳴生活障礙指數亦從92分，減為18分、14分，耳鳴由重度殘疾改善為無殘疾。聽力及耳鳴症狀有明顯改善，對於耳鳴、耳聾患者而言，中醫療法是一個值得推薦的選擇。

A 45-year-old female has had left tinnitus for more than 5 years. One year ago, hearing began to weaken in her left ear and continued to deteriorate from 27 dB, 53 dB, 67 dB, 82 dB, 87 dB to 92 dB within 3 months. However, no abnormalities were identified by X-ray, EEG, and MRI examinations. She suffered palpitation, gastrointestinal discomfort caused by the medications for hearing loss, and her hearing continued to deteriorate. Six months ago, she stopped taking the standard medicine and visited Traditional Chinese Medicine (TCM) clinic to receive herb medicine treatment. After treated by Xiao-Chai-Hu Tan as main remedy for 3 months, hearing of her left ear recovered from 92 dB to 88 dB and 80 dB. Her score in the tinnitus handicap inventory scale (0~100) also reduced from 92 to 18 and 14. We concluded that Xiao-Chai-Hu Tan treatment improved her problem her hearing loss and tinnitus. It is worthwhile to recommend TMC as an alternative treating method for the patient with tinnitus and hearing loss.
