某大學護理學系四年級學生家庭護理實習經驗的反省深思

Students' Reflection of Family Nursing Experience in a Baccalaureate Nursing Program

摘要


本研究為質性研究,目的在暸解某大學護理學~學生修習社區護理學實習過程中,對家庭護理經驗的省思,以做為協助護生家庭護理實習之參考。本研究於50位護生實習結束後,藉由完成家庭護理評值表,暸解護生在歷經二個半月,至少四次家庭訪視之家庭護理經驗(包含照護之家庭類別、過去訪視經驗與本學期訪視次數),並對所經歷之困難與挫折,以及家庭護理帶給護生的收穫與體會進行反省深思。結果發現訪視之素家類別以慢性病居多(30%);護生曾有訪視經驗者有38人(76%),其中多數於修習產科護理學時即有訪視經驗(73.7%);本學期訪視次數以四次居多(52%);訪視過程曾更換葉家訪視者有24%。而針對護生反省深思的內容分析後發現,護生遭遇的困難與挫折,於關條建立期主要來自對棄家情況的不確定成(色含葉家成員、環境及素家與護生互動的態度),以及護生不確定自己能否與棄家達成有效的互動;照護期則包含關條建立期所經驗之困難與挫折的延續、無法有效掌握素家護理問題以及無法有效介入護理措施;結束期則是護生情成涉入過多以及對棄家之護理照護未能有其體成果而成遺,戚。家庭訪視帶給護生的收穫,主要為家庭護理能力的增進及自我認識與成長,本研究相關發現對於未來家庭護理教學應有助益。

To explore senior nurse students' reflection of family nursing experience in the course of community health nursing, a qualitative research was conducted. We collected data at the end point of the course with a semi-structured evaluation form of family nursing experience, in which students were asked to write down their reflection regards difficulty, frustration and what they had learned and thought in the experience of family nursing. There were fifty students agreed to participate the research. Results showed 30% of the family visited had client with chronic disease. Seventy six percent of student had home visit experience before the course. In the stage of relationship establishment, sense of difficult and frustration came from uncertainty of the context of family and ability of self in interacting with family. In the stage of providing care, they ref1ected difficult existed if relationship still could not established. Besides, sense of frustration came from difficult to identified nursing problem and intervened the problem effectively. To withdraw the emotion involved and realize outcomes of family care were not positive enough brought the sense of frustration in the stage of termination. Students expressed what they have learned in care of family were the improvement of family nursing ability and sense of more being. Results of this study could be used as a reference to refine strategy in teaching family nursing.
