建構精神衛生護理師藥物治療護理能力
Constructing the Clinical Competence of Psychiatric Mental Health Nurses-Psychopharmacological Therapy
摘要
本研究的目的在於建構精神衛生護理師之藥物治療護理能力的培訓內容，以提昇臨床護理的實務能力。採行動研究法，第一階段爲準備期：旨在邀集精神科護理主管及教師組成核心小組，形成理念共識，擬定藥物治療爲臨床實務能力項目之一，精神衛生護理師之培訓內容重點，並通過精神衛生護理師甄審委員設置辦法。第二階段爲形成期，建立藥物治療護理能力之培訓內容、種子與實務團隊之組成與運作，研究結果：建構完成藥物治療護理能力的培訓內容，包括處理藥物醫囑完整正確、藥物之相關知識、正確給藥、了解藥物治療過程、熟知藥物作用對病志症狀的影響評估及護理處置（含熟知對藥物副作用的評估及護理處置、了解調整藥物原因及護理處置）、預防藏藥、處理拒藥及藥物護理指導等藥物治療護理需具備的能力指標，以及其定義、行爲操作及注意事項等，期能發揮精神衛生護理師之臨床實務能力，作爲現劃本土性精神衛生護理師培訓與認證制度之參考。

The purpose of this study was to establish the clinical competencies of psychiatric mental health nurses in the field of psychopharmacological therapy, in order to promote the quality of psychiatric nursing care. The method adopted in this study was based on action research. The research process began with the preparation stage and ended with the formation stage. The preparation stage involved: (1) inviting psycfiatric mental health nursing experts and scholars to serve on a core committee; (2) forming a consensus, discussing contents and guidelines and identifying the clinical competencies involved in psychopharmacological therapy; (3) establishing an accreditation committee for psychiatric mental health nurses with the cooperation of two nursing associations. The organization stage involved: (1) constructing the clinical competence content of psychopharmacological therapy for psychiatric mental health nurses; (2) building a seed and clinical practice team in order to test and revise the proposed clinical competencies. The results showed that the major competence indicators and protocols of psychopharmacological therapy had been identified. The following issues were addressed by the clinical competencies. Implementing the order correctly, sufficiency of psychopharmacological knowledge, giving drugs correctly and in accordance with nursing standards, understanding the psychopharmacological therapy process, managing patients with poor drug compliance, and nursing education for patients and families. The development of clinical competencies for psychiatric mental health nurses could enhance the skills of such nurses and provide specific guidelines for clinical workers in the field of psychopharmacological therapy to further developments toward professionalization.
