運用Watson關懷理論照顧一位氣喘幼兒之護理經驗

Applying Watson's Caring Theory to Nursing Care of a Toddler with Asthma

摘要


本文是運用Watson關懷照護的概念,協助一位1歲6個月幼兒期兒童於急性氣喘發作之護理經驗。在護理過程中藉由觀察、會談、治療性遊戲及參閱病歷等方式收集資料,並運用整體性護理評估,歸納出個案之主要健康問題有三項:一、低效性呼吸型態/因氣喘疾病導致過度換氣;二、害怕/對接觸陌生人、環境及噴霧治療感到害怕;三、照顧者角色緊張/因案母不熟悉氣喘照顧,呈現緊張及無助感等。護理期間運用關懷照護的精神與個案及案母建立良好人際關係與信任感,根據幼兒期身心發展特質提供治療性遊戲,協助個案緩解害怕的情緒,並能接受噴霧治療,使個案氣喘狀況緩解;增能案母熟悉照顧要點及緊急處理方式,減輕案母之照顧壓力,進而能協助安撫個案害怕的情緒及身體不適,使之能如期順利出院。

This article describes the application of Watson's Caring Theory in nursing care of an eighteen-month old toddler with acute asthma episode. Health data was collected through observation, interviews, therapeutic play and the patient's medical record. The major health problems of the child with asthma include (1) ineffective breathing pattern, (2) fear of strangers, unfamiliar environment and treatment procedures, (3) caregiver's role strain. Nurses applied age appropriate therapeutic play to relieve the child's fear toward inhalation treatment. Building a trust relationship between child's mother and healthcare providers facilitated the teaching of asthma care and emergency management. The knowledge gained on this disease management empowered the mother to calm the child's emotional and physical discomfort.
