照顧一位老年患者罹患肺炎併發深部靜脈栓塞之護理經驗
Nursing Experience with a Pneumonia Elderly Patient with Deep Vein Thrombosis
摘要
本文為照護一位老年患者罹患肺炎併發深部靜脈栓塞之護理經驗，照護期間自93年5月18日至93年6月15日，筆者為其主護護士，照護期間以系統性觀察及會談，並以歐倫自我照顧理論評估，確立其護理問題有：低效性呼吸型態、現存性感染、組織灌流改變／週邊、營養狀況改變／少於身體需要、身體活動功能障礙及焦慮等六項，並依問題訂定整體護理計畫給予護理措施。過程中筆者發現病患左下肢再次出現疑似深部靜脈栓塞之徵象，經由敏銳觀察及立即反應後，協助醫師迅速確立診斷，在整體醫療團隊合作下，給予積極治療及護理措施，使病患之病況得以改善，未引發致命的肺栓塞，期望藉由此個案提供護理人員臨床照護之經驗分享。

This case report describes a nursing experience of taking care of the elderly patient of pneumonia complicating by deep vein thrombosis from May 18, 2004 to June 15, 2004. The author is the major nurse of the patient. During the nursing, she utilized systematic observation, interview, and Orem's self-care theory assessment, summarize nursing problems: ineffective breathing patterns, existing infection, altered tissue perfusion / peripheral, and altered nutrition/ less than body requirements, physical mobility impairment, and anxiety, then made an entire project according to problems to offer nursing intervention. In process, the author observed the left lower limb of the patient appeared doubtful deep vein thrombosis again. After nursing penetrative observation and immediately diagnosis, we offered active treatment and nursing intervention, and the patient getting improved and not to become fatal pulmonary embolism under the cooperation of whole medical group. Hoping this case would provide experience to nursing staff.
