中期照護效益及其相關因素之研究

摘要

本研究目的乃針對急性疾病治療完成個案，實施跨領域團隊整合性照護，藉以比較個案接受中期照護前後的整體效益及其相關影響因素。採次級資料分析法自「中期照護個案資料管理系統」，汲取2011年1月至2012年12月間共207例，平均年齡為79.9±10.1歲，研究顯示個案接受中期照護服務前後，整體功能各個面向、生活品質及健康狀態皆有明顯進步；而其影響因素則發現性別對個案生活品質，婚姻狀況對工具性日常生活功能，及個案教育程度對心智狀態有影響。中期照護為一具有明顯成效的服務模式，建議未來中期照護發展模式除了善用現有醫療體系與長期照護體系資源，各類專業人員團隊合作外，應藉由完整的照護效益評估與掌握影響效益因素，便可有效的為後續處置做初步評估，進而增加個案整體照護品質，方能提供急性後期銜接性照護，並將資源做最有效率的整合運用，讓中期照護臻於完善，提供更具周全性的醫療照護服務。

This study is focused on providing an integrated care to patients who have completed treatment for acute illness and examined the overall effectiveness and related factors in patients before and after the intermediate care. 207 patients, from January 2011 to December 2012 with an average age of 79.9±10.1, were selected from the ”intermediate care case data management system” by secondary data analysis. The results show the overall functions of the patients were improved in every aspect after the intermediate care. The quality of life and health condition were also significantly improved. It is observed that the patients' gender affects the quality of life, marital status influences the instrumental activities of daily living and the mental status is affected by their educational status. The intermediate care is a significantly effective service model. It is suggested that this model should, in the future, utilize the existing health care system and long-term care resources and the teamwork of professionals and perform completed systematic assessments to improve the quality of care and to provide post-acute care. The intermediate care should effectively utilize the resources to provide a better and completed health care.
