跨功能疾病照護團隊中知識整合與溝通協調關連性研究─以北部某醫學中心為例

摘要


隨著全球經濟發展及生活型態改變,人類出現糖尿病、心血管疾病之盛行率皆已明顯攀升且罹患心臟血管疾病者,有日益增加與年輕化的趨勢;如何早期診斷、有效治療,更進而做到防患於未然的全面預防,已成為不容忽視的課題。在現行的醫院評鑑制度之下,新制醫院評鑑精神回歸以病人為中心、以病人的醫療服務品質取勝,重視醫療團隊的整體合作,但在一般醫院的組織架構下,醫療科較少有跨單位的醫療專科整合,尤其以跨內科以及外科體系的單位整合並以特定疾病的醫療照護團隊更屬少見。由於醫療照護的高度專業性及變異性,應該將各種醫療知識藉由溝通加以整合為醫師所建議的最適合模式,才能真正以病人為中心,將最佳的醫療品質回饋給病人。 本研究目的在於了解某醫學中心所屬的冠狀動脈疾病照護團隊,其知識整合與溝通協調之關連。本研究發放100份問卷,回收有效問卷78份,利用SPSS統計軟體,進行描述性統計分析、相關分析、變異數分析與迴歸分析來驗證本研究之假設。 本研究主要結果顯示: 一、知識整合與溝通協調有正向相關。 二、知識原則與溝通協調頻率、資訊分享、溝通網路、溝通技有正 向相關。 三、是否擔任主管職對溝通網路有差異。 四、是否擔任主管職對溝通協調的差異,並不顯著。 五、是否擔任主管職對知識整合的差異,並不顯著。

Along with global economic development and lifestyle change, the prevalence of diabetes mellitus and cardiovascular diseases was notably increased. It is important to diagnose early and treat effectively. Under the current system of hospital accreditation, the accreditation spirit has returned to patient itself. The main focus is the quality of health care and coordination between different medical departments, but it is still less in general hospitals now and even rare in cross-functional team between internal medicine and surgical departments. The main purpose of this study is to analyze the correlation between knowledge integration and communication of a coronary artery disease caring team in a medical center. In this study, 78 valid questionnaires were collected from 100 subjects and analyzed by Statistical Product and Service Solutions (SPSS) statistical software. The descriptive statistical analysis of correlation analysis, Analysis of variance (ANOVA), and regression analysis were used to verify our hypothesis. In conclusion, 1.Knowledge integration is positively correlative with communication. 2.Knowledge of principles is positively correlative with the frequency of communication, information sharing, communication networks, and communication technology. 3.Being a supervisor is statistically different in communication networks. 4.Being a supervisor is not significantly different in communication. 5.Being a supervisor is not significantly different in knowledge integration.
