成年早期子女經歷父母罹癌對親子間親密關係與生活品質之影響

摘要

研究背景及目的：癌症被視為是一種慢性疾病，醫療照護也從醫院延伸至家庭，因此只要家中有人生病，整個家庭都會受到影響，即所謂「漣漪作用」(ripple effect)。除了患者本身外，其他家庭成員受到的衝擊也將影響其生活品質與親密關係，故本研究欲探討成年早期子女經歷父或母罹癌對其親子間親密關係與生活品質之影響。

研究方法：本研究由電子佈告欄、醫院門診、民間機構等來源，招募126位經歷父或母罹癌的子女為受試者，依親子配對關係分成父子、父女、母子、母女四組。分別填寫個人基本資料問卷、精神依附測量問卷、米勒社交親密感量表與台灣簡明版世界衛生組織生活品質問卷，以皮爾森積差相關、t考驗、變異數分析與集群分析等統計方法進行資料分析。

研究結果：子女性別在親子間親密關係得分未達顯著差異，而是罹癌患者的性別將影響子女自評與罹癌父或母之親密關係。在生活品質上，兒子經歷父或母罹癌時，自評其生理健康生活品質較女兒佳，此外，心理、社會關係與環境範疇之生活品質雖在不同親子配對間未呈現顯著差異；若與台灣簡明版世界衛生組織生活品質問卷所建立一般人常模比較，則可發現本研究受試者在生理健康、心理、社會關係之生活品質皆顯著低於常模，且有12.7%-38.2%的子女生理健康、心理、社會關係之生活品質差且達臨床關注程度。過度投入型依附風格自評親子間親密關係優於安全型與疏離型依附風格；安全型依附風格自評生活品質之心理、社會關係範疇優於疏離型依附風格。

結論：本研究嘗試以量化方式研究經歷父母罹癌對成人早期子女的影響，發現此歷程將對子女生活品質造成負面影響，將有助於助人工作者對於此群體之生理、心理、社會等因素的瞭解，並作為後續處遇或介入之參考依據。

Background and Purpose：Cancer is considered a chronic disease, health care is also extended to the family from the hospital, thus one of family member was sick, the whole family will be affected, the so-called "ripple effect". In addition to patients themselves , the impact on other family members will also affect their quality of life and intimacy. Therefore, this study is to explore the experiences of pre-adult children of parents with cancer of their intimate relationship between parents and the impact of quality of life.
Methods：This study totally collect 126 participants from Bulletin Board System (BBS)、Hospital outpatient and non-governmental organization,which were pre-adult children undergoing parent's cancer,and divided participants into son of father with cancer、son of mother with cancer、daughter of father with cancer and daughter of mother with cancer. Subjects were required to complete personal data questionnaire、Psychosis Attachment Measure (PAM)、Miller Social Intimacy Scale (MSIS) and WHOQOL-BREF. Data collected for statistical analysis of Pearson correlation、t-test and ANOVA.

Results：Children sex in intimate relationships between parents and children scored below the significant differences, but the gender of patients with cancer will affect their children assessing intimacy between a parent with cancer. When children undergoing parent’s cancer,sons get score on physical of quality of life higher than daughters. In addition, psychological, social relationship and environmental aspects of quality of life, although did not show significant differences between four parent-child pairs.If compare scores with the norm established by WHOQOL-BREF, can be found subjects in physical health, psychological, social quality of life were significantly lower than the norm, and 12.7% -38.2% children of physical、psychological and social relationship aspect of quality of life was singificantly poor. Preoccupied attachment style children feel more intimacy between parent with cancer than secure and dismissing attachment style children. Secure attachment style children self-assessment of psychological and social relationship of quality of life were better than dismissing attachment style children.

Concusion：This study tried to measure intimacy and Quality of life of pre-adult children undergoing parent's cancer through quantity method, and found this process will adversely affect the Quality of life of children. This found of study will help clinical professionals understand physical、psychological and social factors of pre-adult children undergoing parent’s cacer, and as a follow-up treatment or intervention of the reference.
