癌末病患之便秘護理問題處置分析-以某醫學中心為例

摘要

在癌症末期的病人中有40％以上有便秘現象，特別在使用嗎啡類藥物的病患可高達90％，且大約75％的癌症末期患者需要肛門栓劑、灌腸、甚至以手挖取的方式來處理便秘的問題。本研究主要針對有便秘問題的癌末病患進行分析，統計中台灣地區某醫學中心癌末病患的排便處置情形，探討癌末病患排便處置與社會人口學、便秘導因、便秘特徵的相關性。收集自2008年1月至2009年1月間，曾有便秘問題的癌末病人之住院病例，進行回溯性分析，共有47名病患。分別記錄排便處置資料；相關的臨床因子則包括活動量少、飲食內容不當、藥物、疼痛、心理因素、排便費力、排便次數少於正常、可觸摸到硬塊、脹氣、個人習慣、生活型態、個人習慣、生活型態、人口統計學等資料。利用本研究所採用的歷史病歷資訊，蒐集完整的護理紀錄，提供與病人相關的資訊，根據資料提供建議。期望在未來能作為新進護理人員的護理措施參考方向，讓護理人員在執行護理措施時有較正確的概念。

More than 40% of terminal cancer patients have the phenomenon of constipation, especially in the use of morphine-type drugs can be as high as 90% of patients, and about 75% of terminal cancer patients need anal suppository, enema, or even a way to hand dig treatment of constipation problems. This study focused on a problem of constipation in terminal cancer patients to analyze in a Medical Center in Taiwan. To discuss correlation of disposal of constipation cancer patients and the social demographic, constipation due to lead, constipation features. We backtrack collect a total of 47 terminal cancer hospitalized patients with constipation problem from
January 2008 to January 2009. Related clinical factors were recorded include the activity less, the contents of improper diet, drug, pain, psychological factors, defecation effort, less than the normal frequency of defecation, palpable lump, flatulence, personal habits, lifestyle, personal habits, lifestyle, demographic and other information.This study provided recommendations according to information of collecting a complete record of caregiver and patient-related information. Look forward to new nurses as a reference to the direction of procedure for nursing care in the implementation of a more concept.
