頷下腺腺樣囊狀癌局部復發及遠處轉移病例報告

Local Recurrence and Distant Metastases from Adenoid Cystic Carcinoma of Submandibular Gland

摘要

腺樣囊狀癌(Adenoid cystic carcinoma)雖為一少見的頭頸部惡性腫瘤，好發於唾液腺，但是確為唾液腺常見的惡性腫瘤。常見於上顎區小唾液腺、腮腺、頷下腺。本文報告一則發生於頷下腺之腺樣囊狀癌。文內敘述腺樣狀癌之生長特性、影響預後因子、治療方法。患者為六十歲男性，於八十七年十一月至本院就醫，臨床上可發現左側下頷臼齒後區可發現一腫塊。經切片證實為腺樣囊狀癌局部復發並懷疑肺部轉移。八十七年十二月患者接受腫瘤泛切除手術、半下頷骨切除術合併頸部淋巴結廓清術。後因腫瘤細胞轉移至肺部，造成嚴重肋膜積水，患者於五月廿三日，呼吸衰竭、心跳停止而死亡。臨床上針對腺樣囊狀癌局部的治療方法包括外科手術切除，放射線法。對於遠處轉移，目前雖無有效方法治療，但因腫瘤生長速率緩慢，腫瘤之局部復發仍應以外科手術合併於射線療法處理，以減低患者之症狀及恢復功能。

Adenoid cystic carcinoma is a rare malignant tumor in head and neck region. It mostly occurs in the major salivary glands and also relatively frequently in minor salivary glands, particularly in palate. This clinical characteristic of this tumor is slow growing; patients may not be aware of the lesion for months and even years in some instances. This study reports a case of adenoid cystic carcinoma of anterior mouth floor s/p operation, with later local recurrence, lymph node metastasis and lung metastasis. This paper will discuss the growth characters, risk factors and the current treatment modality of adenoid cystic carcinoma.
