照顧一位多發性硬化症復發患者之護理經驗
Nursing Experience for a Patient Suffering from Recurrence of Multiple Sclerosis
摘要
多發性硬化症是一種中樞神經系統疾病，會造成視力喪失、肌肉無力等症狀，大多發生於年輕女性。本篇敘述一位39歲已婚婦女，經歷多發性硬化症三次的發病，期間曾經服藥自殺，之後至復健科病房行復健治療的照護經驗。筆者為個案之主護護理師，於2009年2月16日至3月23日護理期間，藉由直接照護、觀察、互動溝通，並運用Gordon十一項健康功能型態評估及個案討論等方式收集資料，發現個案的主要護理問題有：潛在危險性感染、身體活動功能障礙、慢性疼痛、潛在危險性自我殘害等四項。筆者針對問題原因，給予個別性護理措施，滿足其身心需求，並鼓勵表達自我。個案經由持續的照護，除護理問題獲得解決外，也能說出心裡的感受，勇敢面對自己未來的復健之路。希望藉由此篇護理經驗分享，提供臨床護理人員日後照護類似病患之參考。

Multiple sclerosis is a disease of the central nerve system. It can result in vision loss and muscle weakness, and mostly occurs in young women. The article reports our 16th February to 23th March 2009 nursing experience caring for a 39-year-old woman who had a recurrence of disease and had attempted suicide prior to rehabilitation. Using Gordon health function to make the evaluation and case discussion, as well as observation, interview, and direct caring to collect information. She was found to have potentially dangerous infection, physical disability, chronic pain, and potential risk of selfmutilation. The writer provided with individualized nursing care to meet her physical and mental needs and encouraged the case to express herself. Through continuous care, she would accept and resolve her problems, become more resolute in facing her nursing problems. We became better at communication, express her feelings, and bravely face her path of rehabilitation in the future.
